Compassionate holistic care retains Latina trans women in HIV prevention services

A qualitative study has shown that having peer workers and adequate training of staff at all levels are key to ensuring trans-affirming care and retention of trans women in HIV prevention services. Latina transgender women are best served and engaged when they are given holistic care and services beyond HIV prevention, say Dr Sophia Zamudio‐Haas of the University of California San Francisco and colleagues.

6. Nuevos tratamientos

El presente año 2022 no ha sido especialmente prolífico en lo relativo a nuevos tratamientos antirretrovirales. En todo caso, las noticias se han repartido principalmente entre la aprobación de lenacapavir (Sunlenca®) y la vuelta a la investigación de islatravir tras una interrupción de los ensayos clínicos el pasado año.

Respecto a lenacapavir, el pasado mes de septiembre la Comisión Europea aprobó su comercialización para el tratamiento de la infección por VIH-1 multirresistente –en combinación con otro u otros antirretrovirales– en adultos con infección que se encuentren en un régimen de tratamiento antirretroviral fallido por causa de problemas de tolerabilidad, resistencias o aspectos relativos a la seguridad. Un estudio publicado tras su aprobación europea mostró la robustez del fármaco –de administración semestral- y concluyó que su eficacia no se vería afectada por edad, sexo, etnia o niveles de CD4 basales.

New HIV drugs should be classed as inferior if they carry a higher risk of resistance

New antiretroviral regimens should be considered inferior to older ones unless trials can demonstrate that their failure doesn’t lead to more drug resistance than standard treatment, Italian researchers argue in Lancet HIV. They say that recently completed trials of injectable cabotegravir and rilpivirine raise concerns about the risk of developing resistance to two drug classes if the regimen fails.

Free antiretroviral therapy for non-citizens in Botswana: a further step towards the elimination of HIV

Following the expansion of antiretroviral therapy to migrants and non-citizens in Botswana, gaps have narrowed in the uptake of antenatal care and antiretroviral therapy during pregnancy between citizens and non-citizens living with HIV. Disparities in adverse birth outcomes were no longer observed after the change in policy, according to a report in the Journal of the International AIDS Society.

How do participants feel about analytical treatment interruptions in HIV cure-directed trials?

Experiences of participants in a US HIV cure-directed study with an extended analytical treatment interruption are reported in HIV Research & Clinical Practice. The researchers recommend that those designing HIV cure-directed trials should manage participants’ expectations and focus on the contribution they are making. Providing psychosocial support to participants must also be considered, as well as preparing them for the mental health impact of viral rebound.

“With the right policies, regulations, and technical guidance,” the volume of HIV self-testing kits sold in Uganda was multiplied by twenty, Fosca Tumushabe from Population Services International told the 22nd International Conference on AIDS and STIs in Africa (ICASA 2023) last week in Harare, Zimbabwe. In addition, the volume of non-approved kits sold more than halved, while close to two million people were reached with HIV self-testing information through digital channels.

Tuberculosis still much more common in people with HIV in high-income settings

Tuberculosis is still one of the world’s deadliest infectious diseases, with 10.6 million people developing active (symptomatic) TB infection in 2022 and 1.3 million dying from it. That’s second only to COVID, which killed 6.95 million people last year, and far more than the 630,000 who died due to HIV. But it still receives less publicity than HIV – even though it is the greatest cause of death on people with HIV, with more than a quarter of HIV-related deaths (167,000) directly due to TB, and not entirely in immunocompromised people.

Heavy drinking, metabolic disorders and hepatotoxic antiretrovirals are risk factors for chronic liver disease in UK cohort

More attention should be paid to modifiable risk factors for chronic liver disease in people with HIV, such as alcohol, metabolic disorders and being overweight, HIV and hepatology specialists in Brighton, England report in the Journal of Acquired Immune Deficiency Syndromes. In a study of people with HIV who had abnormally high liver enzyme levels, they found that one in five had clinically significant liver damage and 60% had risk factors that could be modified by treatment of metabolic complications and/or abstinence from alcohol.
